  • Homeschool Full Report - Online Form
    Revised August 29, 2011
    This form is to be printed and filled out or submitted electronically to resident school superintendents by October 1 of each year, or within 15 days of withdrawing a child from public school in order to homeschool.
  • Letter of Intent to Continue Homeschooling - Online Form
    Revised August 29, 2011
    This form is to be printed and filled out or submitted electronically to the resident school superintendent by October 1 of each year after the instructor has provided the same district with a Full Report.

Reporting

Help for Filling out the Compulsory Report Form
Nothing in Minnesota law specifies exactly how non-public educators are to make their annual report only that it is due by October 1 each year and certain information is to be communicated. You are free to write a letter of your own, use this MACHE designed reporting form, or some other device.
